Many people avoid orthodontic treatment because they do not want to show braces when they smile and talk. In Lingual orthodontics the braces are placed on the lingual surfaces of your teeth (internal) so no one can see it. This way you can continue to smile and speak freely through out the treatment.
The above picture shows a case of lingual orthodontic treatment. As you can see the bottom picture, the braces are not visible when smiling or talking because they are placed on the lingual (internal) part of the teeth.

Can I have lingual orthodontic treatment?
The Lingual orthodontic systems are particularly suitable for adults and for permanent dentition.
Lingual orthodontics can also be suitable for adolescents with few conditions: the permanent teeth have fully erupted (in case there is an impacted tooth then all the permanent teeth should have erupted excluding the impacted which will be treated), and the lingual surfaces are large enough to allow for placement of lingual brackets.

What is Incognito?
Incognito (also called iBraces in the USA) is a particular system of lingual orthodontics. In this system, brackets are placed on the lingual (internal) surface of the teeth which is not visible. The lab procedure for incognito is an integral part of the treatment. According to the orthodontist’s instructions of treatment plan and final tooth position the wires and brackets are especially built to fit the specific treatment needs. This individual fabrication of the braces and wires allows for good accurate results.
Not every orthodontist can practice this system. Using this method requires special training and qualification.

Advantages of the Incognito lingual system
- The braces are invisible and not seen when smiling or speaking.
- The thin profile of the brackets is comfortable to the patient.
- Because the brackets are placed on the lingual (internal) surface of the teeth, the risk for white spots is low and they can not be seen.
- The lingual location of the brackets allows the patient and the orthodontist to see the progress of the teeth and the smile clearly without interference of the orthodontic equipment.
- The Brackets in the Incognito lingual system are composed of an alloy which contains a high percentage of gold and therefore may reduce the risk for irritation of the gingival tissue.

Limitations of lingual systems
Oral hygiene is particularly important as it is difficult to brush with brackets. Lingual orthodontics is appropriate for those who can maintain good oral hygiene.

- Clear brackets: there are several types of external porcelain brackets or plastic tooth that simulate the tooth color so they are very pleasant in smile and barely visible. Brackets are clear or tooth colored.
- Clear aligners such as Invisalign – orthodontic tooth movement is achieved by a series of clear removable alligners. In this method the equipment is also almost invisible.
